All Filipinos are required to report birth of their children, change of civil status such as marriage, which the Philippine Embassy in Canberra forwards to the Philippine Statistical Authority (formerly NSO). The Philippine Consulate office in Brisbane is required to notarise your Report of Birth/Marriage Form, the Applicant is then responsible to send all other required documents along with notarised Report of Birth/Marriage Form directly to the Philippine Embassy in Canberra. At your appointment please ensure you bring your:- • TWO (2) Forms of VALID GOVERNMENT ID's that would meet the 100pt ID requirement such as: - passport (Dual citizens are advised to bring Philippine passport - citizenship certificate/card - driver's license - birth certificate - Senior's card - Medicare card - Philippine government issued ID NOTE: The government ID should be valid and should support the claim of citizenship on the document. • FOUR Copies of FILLED OUT BUT UNSIGNED Report of Birth/Marriage form (to be notarised at our office) • Processing fee for notarising your Report of Birth/Marriage form of AU$45 (cash only) • Self-addressed Express post Envelope for processing and release by mail within 1-2 business days The Consulate assumes no responsibility for any delay, loss or damage in the mail or while the documents are in custody of courier service.
